Full physicals are required by Illinois Law for:
- All children entering Preschool
- All children entering Kindergarten
- All children entering 5th grade
- All children entering school for the first time
- All children transferring from another state or country
Immunizations
All new students must have an up to date immunization record. They must be filled out, signed by the doctor and returned to the school office.
Dental Examinations are required for Kg, 2nd and 6th grades.
School Exclusion: According to Illinois Law, any child whose parent has not presented proof of the required health examination and immunizations shall be excluded from school. Parents shall meet these requirements by the FIRST DAY OF SCHOOL. After that date, if such proof has not been presented, the child will be refused admittance to school.
